EXISTING CLIENT RESERVATION REQUESTS   Please fill out all information otherwise we may not be able to complete your request.  Also, note all reservation requests are NOT confirmed until you speak with our office directly as we may not have any availability during your requested time periods. It is very important to book your reservations EARLY as we do fill up.  As an existing client we need minimal information (and any information that has changed) for a reservation request.  

TIME-SAVING CHECK-IN FORM:  In addition to making an online boarding reservation request - you MUST print and complete the Pet Care Services Agreement to save some time when checking in at Bed Bath n Bonz.  Pet Care Services Agreement    The Service Agreement form is our boarding contract, which is a required form that must be filled out to complete the registration process. You can either fax this form to us at 903.894.9305 or bring it in when you check in along with shot records which also must be either faxed or brought in at check in.
